Rehabilitation Services include Physical, Occupational and Speech Therapies, and are provided in the home by Gurwin’s licensed therapy providers. Often, a health care provider will prescribe this service based on needs after surgery or a hospital stay.
Nursing services include wound care, health monitoring, dressing changes, blood pressure and/or blood sugar monitoring, and many other skilled nursing services, as needed.
Nutritional counseling includes visits from a certified dietician, who will coordinate with other professionals to create a diet appropriate for each patient’s needs.
Social work services are provided as needed, including mental health assessment, social services counseling, and other applicable services.

Personal Care Services
When a patient has recently returned home from a hospital or nursing care facility or is declining medically and in need of supervisory care, additional support is available through our Licensed Home Care Agency. Our staff at the Agency will discuss specific concerns, and help identify realistic solutions based on individual needs.
Home Health Aides
The Gurwin Home Care Agency provides Home Health Aide services from 4 hours to 24-hours per day, one to seven days a week. Our Home Health Aides are carefully screened, specially-trained, and receive ongoing in-service training. They are under the direct supervision of our RN staff.
A customized plan of care is developed at the onset of care, and is used daily to guide the tasks/activities that the aide provides. This may include all aspects of personal care, meal preparation and safety monitoring, and may also include encouraging a client with rehabilitation exercise as directed by a physical therapist.

Companion Services
Companion services are one-on-one services that provide our clients reassurance by having an individual present in the home or in an acute setting such as a hospital or nursing and rehabilitation center. The companion may act as an advocate for the patient by requesting assistance, but at no time may a companion provide “hands-on” care. Our companions engage in conversation with clients and participate in activities such as reading or playing cards, and are available to call for help should the need arise. Companions are present when family members cannot be there, providing peace of mind and a friendly face. Dependent upon a client’s condition, our companions may be able to provide assistance in getting a client to the bathroom, as well as assisting with feeding, if necessary.

Frequently Asked Questions: Home Care Services on Long Island
How do “live-in” services work?
Our live-in home health aides often stay with the patient for three to four weeks at a time and then leave for a few days. A substitute live-in aide is sent for those days until the “permanent” live-in HHA returns. The live-in aide does need a private area in the home for hours of down time and sleep, generally 8 hours for sleep and another 3 hours for meals and break time. If the HHA needs to get up once or twice to assist with toileting during the night, that is acceptable.
What tasks can a Home Health Aide assist with?
We call the tasks ADLS, or aid to daily living. This includes assistance with bathing, showering, oral care, grooming, dressing, meal prep and reminding to take medications. The home health aide (HHA) also provides for safety monitoring, does laundry and light housekeeping for the patient, and provides companionship. Physical and Occupational therapy are available if needed. A physical or occupational therapist will visit you at home to complete an evaluation. Based on the results, you will be notified of the goals expected from home therapy and a set time to complete it.
