Who Is Homeward Bound For?
Homeward Bound is designed for adults who have recently completed or are nearing the end of a rehabilitation stay and need additional time and support before transitioning back to their permanent residence. You don’t have to be a Gurwin rehab patient to participate. Homeward Bound is open to anyone who qualifies, including residents of other rehabilitation communities who are not yet ready to return home following a hospital stay.
A clinical assessment is required prior to admission to ensure the program is the right fit for your needs.

Homeward Bound participants stay at the Gurwin ~ Jewish Fay J. Lindner Residences in Commack, enjoying all the comforts and amenities of our assisted living community for the duration of their stay. Each resident receives an individualized care plan tailored to their specific recovery goals, with access to a full range of services:
- Fully furnished apartment with emergency call system
- Individualized care plan
