Gurwin Jewish Nursing & Rehabilitation Center is an award-winning, not-for-profit, non-sectarian health care facility located on a 36-acre campus in Commack, Long Island.  Our 460-bed Skilled Nursing Pavilions provide long-term skilled nursing care; short-term rehabilitation; sub-acute medical and post-surgical care; ventilator and respiratory care; memory care; an award-winning social and a medical model adult day health care program; and palliative and hospice care.

Our beautiful community and caring staff create a comfortable home for residents, but as importantly, our facilities are unlike those at any other skilled nursing community on Long Island.  The Gurwin nursing and rehabilitation center features a 12-station dialysis center, on-site pharmacy, two well-appointed rehabilitation gyms, an award-winning virtual reality memory care Immersion Room and robust social programming, and is staffed by a wide array of clinicians including physicians, social workers, nurses, occupational/physical/speech/recreation therapists and other health care professionals to help residents thrive and enjoy a full quality of life at Gurwin. Everything they need for health and well-being is readily available in our state-of-the-art skilled nursing center, named one of Newsweek Magazine’s Best Nursing Homes.

Skilled Nursing Staff

Gurwin has been providing outstanding nursing care for more than three decades. We provide expertise that other communities may not offer, including a full time medical director, board-certified geriatricians, a licensed social worker on each floor, full-time respiratory therapists, physical, occupational and speech therapists and scores of specialists who provide services to our residents in our on-site clinic.  In addition, we are fortunate to have dedicated community volunteers who donate their time and talents to enhance the lives of our residents.

Rooted in community and built on decades of compassionate care, Gurwin’s story began in the early 1980s when UJA-Federation transformed a Commack School District elementary school property into what would become a cornerstone of Long Island healthcare for frail, elderly and medically complex older adults. Today, those same charming gym floors — preserved in our two state-of-the-art rehabilitation gyms — serve as a living reminder of where we started.

Frequently Asked Questions: Long Term Skilled Nursing in New York

What medical services are available for long-term skilled nursing residents?

Gurwin has physicians available, Monday through Friday from 8 a.m. to 5 p.m. Medical coverage is extended into the early evening and weekends by either a nurse practitioner or physician assistant, with a physician on call. In addition, residents are able to access healthcare services such as optometry, dental, audiology, x-ray and a variety of other specialties through consulting physicians -- without ever leaving our community. Should a visit to a specialist in the community be required, it can be facilitated through the attending physician here at Gurwin.

If a resident needs to go to the hospital, where do they go?

If it is a scheduled need, most long-term skilled nursing residents go to Huntington Hospital, although other arrangements may be made after consultation with the resident and/or family. If there is a need to call 9-1-1, state law mandates the resident be taken to the closest hospital, which is St. Catherine of Siena in Smithtown, part of the Catholic Health System.
