Island Nursing and Rehab Center is one of only a select number of long-term skilled nursing facilities on Long Island with a dedicated team who is highly skilled at supervising this specialized medical treatment.
The Island Nursing and Rehab Center’s clinical team provides specialized nutritional care for medically complex patients with short- or long-term health conditions that impede adequate absorption of nutrients. Total Parenteral Nutrition (TPN) is a life-sustaining therapy delivered intravenously through a central line, providing essential nutrients such as proteins, carbohydrates, glucose, fats, and vitamins and minerals.
For patients requiring TPN therapy, an individualized plan of care is based on a patient’s age, clinical status and laboratory tests results. The multidisciplinary team includes a physician, pharmacist, registered dietician and registered nurse. This team will assess the patients’ progress daily to ensure effective nutritional support, and the patients optimal wellbeing. Medical conditions that are primary indications for TPN include:
