Gurwin Jewish Nursing & Rehabilitation Center partners with Fresenius Kidney Care to operate an on-site dialysis center serving patients across the care continuum, from long-term and short-term residents to assisted living residents, home care patients, adult day health participants and members of the broader Long Island community.
Our hemodialysis center uses current technology in a clean, well-maintained facility, staffed by board-certified nephrologists, registered nurses, and skilled phlebotomists. The center operates six days a week across three daily shifts, offering flexible scheduling that works around each patient’s care plan.
Because dialysis happens right on the Gurwin campus, patients avoid the burden of outside transportation and extended wait times. Continuity of care is maintained through a coordinated team that includes physicians, nursing staff and social workers. For short-term rehabilitation residents, on-site dialysis means fewer disruptions to therapy schedules. Patients stay in a familiar setting, surrounded by staff and neighbors they know.
Community members from the Commack area and across Long Island are also welcome to apply for available dialysis slots.
